Frequently Asked Questions:

Can someone younger than 18 participate?   DPP is for 18+.  17 and under would go to our Kidshape program, which we are currently working on getting scheduled. Check back!

      How do providers make a referral? Ask the family/patient to call?  If Provider wanted to make a referral,  they can send an order for DPP to our fax 888-624-3107 with their name, phone number, and copy of labs if HIPAA release signed.  EA will then call the patient.  Or parent/family can call us as no formal provider order required.

Who is in charge of checking insurance coverage? ExercisAbilities will do all of the insurance checks at our office.  At this time,  all major insurances including Medica, BCBS, Medicare, MN Medicaid, and Ucare cover it with lab-confirmed prediabetes values and 18+ years old.

Qualifications for insurance coverage:

You must be 18 years or older.
You cannot be previously diagnosed with Type I or Type II Diabetes.
You must have a BMI > 24 (>22 if Asian).

And at least one of the following in the past year (need a copy of the Lab report on file):

  • HbA1C: 5.7% – 6.4%
  • Fasting plasma glucose 100-125 mg/dL
  • 2-hour plasma glucose (after a 75 gm glucose load): 140-190 mg/dL
  • Previous diagnosis of gestational diabetes (Not an allowed qualification for Medicare)

When I first heard about this program, I was hesitant due to the gym workouts in a group setting. I have joined gyms in the past and I have never felt comfortable in that environment. I reluctantly signed up and it turned out to be a great decision.
